We accept all dental insurances.  In addition, we also accept Dental Medicaid and Medicare to serve our community.

To save time, please first complete the online forms.  Otherwise, we will require your dental insurance group ID,  subscriber ID, date of birth, first and last name, and customer service number to your dental insurance company.  Also we will require your contact phone and email information as well.  Typically, you will contact after your insurance information is verified.

Click on FORMS at the top of our website.  A security code will be emailed or texted to you.  Please enter the code back to activate your online forms.   We ask each patient to complete one set of online forms and you have to be over the age of 18 to complete the forms.  For your convenience, online forms are presented in a slide show format.  It takes about 20 minutes to complete electronically.

We typically reserve a limited amount of emergency appointments per week.  The sooner you complete the online forms, the sooner we can schedule your appointment.

Most of our patients prefer text.  Please let us know your preferred mode of contact when completing your online forms.
